4 Must be a VISA Elite credit cardholder to receive the USD 1,000,000 term life coverage. UNFCU credit cardholders must enroll in the Clements card-based Term Life program. The 60 days complimentary period is only available to members who enroll in the card-based life program. Automated credit card billing will commence after the 61st day. A UNFCU VISA credit card will be required for payment. Payments will be automatically deducted from your UNFCU credit card every month. To cancel payments, members will need to contact Clements Worldwide at 1 800-872-0067 (toll-free US and Canada) or +1 202-872-0063.

5 Eligible purchases must be made at merchants identified by VISA that are within the supermarkets or dining category. Eligible purchases are charges to your account for goods and services that have not been returned or otherwise credited to your account. Special double points offer is also subject to change.

6 The cardholder must charge the entire amount of the scheduled airline fare to their VISA Elite credit card. Covered reasons of the insured person must be caused by or result from: Death, accidental injury, disease, or physical illness of the insured person or an immediate family member of the insured person (death, accidental injury, disease, or physical illness must be verified by a physician and must prevent the insured person from traveling on a covered trip) Default of the scheduled airline resulting from financial insolvency.

Your trip interruption and cancellation insurance becomes effective on the latest of: the effective date of the policy, the date on which you first meet the eligibility criteria as the Insured Person, or the beginning of the period for which required premium is paid for You. Insurance for You automatically terminates on the earliest of: the termination date of this policy, the expiration of the period for which required premium has been paid for You, the date on which You no longer meet the eligibility criteria as the Insured Person, or the date on which the Federal Insurance Company pays out 100% of the Loss of Life Benefit Amount. 7 With your Elite Card, you are not responsible for unauthorized merchant transactions that are reported promptly (no later than 60 days after the statement date on which they appear). If your card (or card number) is ever lost or stolen, you are protected against unauthorized purchases. You must report your missing credit card immediately or notify us as soon as you detect any unauthorized activity. UNFCU reserves the right to not issue or re-issue your credit card if two (2) instances of fraud on any of your accounts were reported within a period of three (3) years, or if you fail to report a lost or stolen card in a timely manner.

8 Chip and PIN also known as EMV is an international standard replacing the magnetic stripe on bank cards in payment transactions. Most countries implementing smart bank cards require the use of a Personal Identification Number (PIN) in each transaction, which is why chip and PIN is often used to refer to EMV implementation programs. The card can be used both with a magstripe (same as before) and the chip, ensuring that both US and non-US merchants will be capable of accepting the card. PIN transactions are more secure than those requiring a signature, and protect you against fraud. Please note that cash advances are also PIN-based transactions that are subject to finance charges from the transaction date. Making a purchase using your PIN is not considered a cash advance and will not incur interest or finance charges if you pay your balance in full by the due date.
